PACE (Program of All-inclusive Care for the Elderly) is a Medicare program that helps people meet
their health care needs in their homes and community, instead of going to a nursing home or
other care facility.
As described by the NJ Department of Human Services, Division of Aging Services’ website, PACE is
an innovative program that provides individuals who are 55 and older, and deemed “frail,” with
comprehensive medical and social services. These services are coordinated and provided by an
interdisciplinary team of professionals in a community-based center and in their homes. PACE helps
participants delay or avoid long-term nursing home care.
During the webinar, PACE provider Lisa Zavorski, Executive Director of LIFE St. Francis, a PACE in
Bordentown, will present an overview of the PACE model, describe how it differentiates from other
managed care models and explain why this program should be expanded throughout New Jersey.
